At Lynn Community Health Center (LCHC), we are proud to introduce our new Mobile Van initiative as a cornerstone of our Medical Outreach Program.

This innovative approach enhances our commitment to providing accessible and compassionate healthcare services to the most vulnerable members of our community.

Our Journey

The Medical Outreach Program was conceived a decade ago through a partnership between the Lynn Health Task Force and our leadership at LCHC. Recognizing the unique healthcare needs of individuals experiencing homelessness, we began with a simple yet powerful idea: to bring healthcare directly to those who need it most. Our program has grown from humble beginnings into a comprehensive service model, including our 20-bed recuperative care program and street medicine services.

Mobile Van: Healthcare on Wheels

With the introduction of our Mobile Van, funded by the generosity of MGB, we’ve taken our services to the streets of Lynn. The van is equipped with state-of-the-art medical facilities to provide on-demand episodic care, primary care services, and much more. Our Mobile Van stands as a beacon of hope and a testament to our community’s resilience, serving as a mobile health clinic that reaches every corner of the city.

Services Offered:

  • On-Demand Healthcare: Immediate medical care and stabilization, followed by transition to our primary care or Orange teams.
  • Specialized Programs: Low barrier access to BUP treatments including Suboxone, Vivitrol, and Sublocade.
  • Narcan Distribution: Proactive measures to combat overdoses in the community.
  • Comprehensive Case Management: Assistance with housing, identification, insurance, and benefits to support our patients’ overall well-being.
  • Referrals: Seamless connection to detox services, ensuring continuity of care.
