Lynn Community Health Center participates in two programs through MassHealth which coordinate comprehensive primary care and social support for patients and families.

Senior Care Options (SCO)
- For patients age 65+, enrolled in Medicaid and Medicare
- Coordinates primary care, geriatric specialty care, and social support
- Provides respite care for families and caregivers
- No co-pays
- No cost for transportation to medical appointments
- 24-hour access to your care team
OneCare
- For patients age 21-64, enrolled in Medicaid and Medicare
- Coordinates primary care and social support
- Provides respite care for families and caregivers
- No co-pays
- No cost for transportation to medical appointments
