Health Center recognized by DPH

commissioners awardThe Department of Public Health presented Lynn Community Health Center with the  Commissioner’s Leadership Award at the 2013 Ounce of Prevention Conference on October 22.  The health center was recognized for our Integrated Care Project, a unique, fully integrated behavioral and medical health care team model that involves: co-location of behavioral health and primary care services and co-management of patients. Integrated Health Care is a patient-centered approach that delivers point-of-care behavioral health services, minimizes stigma, increases care coordination, and efficiently improves access to needed care, resulting in increased quality and improved clinical outcomes.
