In addition to comprehensive primary care, you may want to consider participating in one of two programs that provide additional supports for seniors.
Program for All-Inclusive Care for the Elderly (PACE)
Lynn Community Health Center, along with Greater Lynn Senior Services, has established Element Care as joint venture to bring Program for All Inclusive Care for the Elderly (PACE) to frail seniors in our community.
If you are 55 or older, and are medically eligible for nursing home care, you may want to consider PACE. This program will give you the support you need to remain safely in your own home for as long as possible. Your integrated PACE team, led by your primary care provider, provides a comprehensive range of preventive, primary, acute, and long-term care services. You may receive services in your home or at one of our Adult Day Health Centers.

Senior Care Options (SCO)
If you are 65 years or older and meet the financial eligibility requirements for MassHealth (Medicaid), you may be eligible to join the Senior Care Options (SCO) Program. This program offers comprehensive health care services, coordinated by your primary care provider at Lynn Community Health Center or our Market Square Practices, along with social support services. You have 24-hour access to care and active involvement in decisions about your health. Your primary care physician and a team of nurses, specialists, and a geriatric support services coordinator will work with you and your family to develop a plan of care to specifically address your needs.
